While I might like to suggest Maskwood Nexus... I don't actually think it helps you considering the lack of non-doctor creatures. It may still be useful though for pushing you over the line to reach 13.
Three Tree City is a card that might be good though.
K-9, Mark I is really good. Your board state is very important to you so forcing people to pay just a little extra to interfere with it might help you edge out a game. But at the same time, several of the doctors have to attack to activate their abilities and this will help them not die when the do so. Psychic Paper and can also do something rather similar, but might also get you that extra doctor you need. Brotherhood Regalia is another similar card.
I like where you're going with Glasspool Mimic Flip, but it wont bypass legendary. Instead Sakashima the Impostor and Sakashima of a Thousand Faces can both help get around this. Even if they aren't also land.
Similarly Mirror Box is good... but doesn't synergise with what you're doing as you don't have many duplicators for it to work with. I feel like you were going a bit more for Helm of the Host?
All in all though, these are just minor tweaks, the question is what part of the game does this deck operate around? Many of the doctors are spread over a lot of the game, which leads to them being quite unfocused. But I feel you are bringing this towards being a very upkeep centric deck? The question is how to maximise that. It will help Gallifrey Stands go off, but will also help your suspended cards and other triggered effects.
Lithoform Engine could help you in several ways, duplicating doctors, but also duplicating the trigger effect of Gallifrey Stands. While not quite as useful, Annie Joins Up will also double the triggers on your doctors. Roaming Throne is also a trigger doubler... though it's a little dull because it's in every bloody deck now.
Paradox Haze similarly gets you that extra upkeep, but it falls into the problem that a lot of such things do... One extra upkeep per turn is good, but a turn is a long time. While it may be the best option, a way to get creatures into play might be a better slot in your deck?
But there is also another two problems all decks have, mana and getting the right cards. Firstly, mana. You have a lot of expensive cards and you want to be able to cast all of them. Lotus Petal is good... but I don't see it being better than a normal mana rock as you have no ability to recurse it for free. Herald's Horn would probably be better as it can remove about 25% off the cost of all your important cards, and lets you take a peek for them. Leyline of the Guildpact can help with mana fixing, though wont actually set you ahead, as would Three Tree Mascot (which also counts as a doctor). Patchwork Banner is a newer mana rock that helps buff your guys a little. Mirari's Wake is also a solid mana doubler, if a little expensive.
Kinnan, Bonder Prodigy is also very cheap and solid, though works better if you have more mana rocks.
You've got a bit of recursion from The Eighth Doctor, which is more likely to turn up due to your commander, but that means that you can take advantage of the search lands. It would require changing out a lot of the land you do have, but if you use the typed dual lands like Canopy Vista or Godless Shrine (because you may need 1 mana more than 2 life), you can search for them and then recurse the search land like Flooded Strand (which could search for any dual land as long as it's part plains or part island). If you want to find what land works like this it's best to google mtg land or some such, there are whole websites dedicated to it. Considering you're running 5 colours, Bloom Tender and Faeburrow Elder can also help supercharge your mana.
As for getting cards, I feel like you've got a decent search to get the doctors, but you are a little lacking in turn to turn draw. This often means you'll end up sitting in a game with only one card to play a turn and that's not fun. The One Ring is another groan worthy staple for how good it is (and therefore in everything), but only really relevant if you're running proxies. Collector's Vault is something I'm fond of, while it wont give you card advantage, it's cheap and you don't really mind discarding doctors anyway. Sylvan Library is also insanely powerful.
A lot of the search you do use is really expensive as well. Conflux and Diabolic Revelation are good... but they each cost a huge amount of mana, after which you still have to actually cast the cards. These are things that might help you on turn 10+ without a lot of acceleration. Instead something to get your creatures directly onto the battlefield like Victimize or possibly Pyre of Heroes.
Anyway, that's enough from me for now. Remember, look for cards that get you to your win conditions. Omniscience is great, but only if you can cast it AND if you've got the cards to play AND they are cards that lead you to victory. Perhaps 1 mana per turn or 2 cards might have been more useful.

Since you are running Mikaeus, the Unhallowed, Triskelion, and Buried Alive I find that it's helpful to have a couple reanimation spells that can get two creatures out of the graveyard at once. Victimize and Living Death are my usual picks.
With Jarad in the deck, Coffin Queen might be worth considering as a way of repeatedly returning large creatures to the board for sac fodder.
Titanoth Rex could be useful in Mimeoplasm. It's a big body that puts itself into the graveyard for cheap, draws you a card, and can give Mimeoplasm trample if it doesn't already have an evasive ability.
Ghostly Pilferer is another potential card for the deck that functions similar to Psychic Frog and Likeness Looter. Cheap, lets you discard, provides card draw, and gives you an evasion option if you end up copying it as your main creature for Mimeoplasm. I like that it lets you passively draw cards any time an opponent casts their commander from the command zone.

Deck looks like a lot of fun to play! You have quite a few inefficiencies but after having read through the comments I see no fault in your build path! Sometimes throwing together a stack of cards you like in a color you like is all it takes to have fun! :)
I would recommend adding some Reanimation spells to the deck! There's some great budget options in things like Victimize and Persist, but also Animate Dead and Reanimate are much cheaper than they once were!

I also don't really understand Fog or Spore Frog but I know you like politics so maybe they'll be fun in order to get favours from people. Loads of card draw which is nice but again I can see you running out of creatures to sac for Village Rites and Deadly Dispute type cards. You generally need to draw cards while behind and while behind you might be suffering from graveyard hate agaist your skeletons or other effects - just something to consider. Also Damnation might be something to consider switching for a different boardwioe if you are gonna put some regenerate creatures in (Toxic Deluge turns off regenerate aswell - but it's such a good card it's probably worth it)
In terms of cuts to make to make space for more dummies, Shadow in the Warp doesn't really fit much in the deck - you have so many cheap creatures that you'll probably be casting more than one a turn and it will rarely help that much - and the 2 damage is pretty insignificant compared to your other effects. I would favour Llanowar Elves type cards over Fertile Ground as again, creatures are what you want and they give you the oppurtunity to sac them if you need.
Some good cards to consider:
Victimize - brings back your Sakura-Tribe Elder type cards and is a sac outlet at the same time
Kroxa, Titan of Death's Hunger - just great value aswell as triggering Bhaal. A good reanimator target aswell.
Recurring Nightmares - Reanimator + sac outlet + it doesn't die? This card's great.
Dread Wanderer? Not the best but helps you out in a pinch if you have a Village Rites and nothing else
Nether Traitor - I don't know why this isn't in there already

Hey Indie,
First off I really like the lands you have assembled and the suggestions from SufferFromEDHD are all good.
You are definitely going to want an artifact ramp package.
Sol Ring, Mana Crypt, Arcane Signet, Fellwar Stone, Gilded Lotus, Thran Dynamo
Special Mention to Nyx Lotus, it could be good being in mono black. and Jeweled Lotus, pays for your commander for free on turn 1 even.
Sword of the Animist is not bad ramp at all and buffs yours rats
Commander's Plate might be cute.
Liliana of the Dark Realms, makes sure you always get a land drop and could make your swamps tap for four.
Chittering Witch if your playing with more opponents.
Crypt Ghast is sort of ramp, but doubles your swamps. Very good in mono black.
Dauthi Voidwalker is not on theme at all but is just sooo much value in a two cost card I have to recommend it.
God-Eternal Bontu is excellent draw power in the right situation.
Dont forget you can run any number of Relentless Rats
Gray Merchant of Asphodel will heal you up a lot and hurt your opponents
Krav, the Unredeemed is also great card draw.
Yawgmoth, Thran Physician, has card draw, removal, and proliferate all on one card
Deadly Rollick, potentially free removal
Defile, cheap good removal
Thrilling Encore saves you from a board wipe or lets you bring everything back after sacrificing it all to like God-Eternal Bontu
Phyrexian Arena some ok card draw.
Bojuka Bog if you are worried about graveyards
Mutilate, a good board wipe in mono black
Night's Whisper, Read the Bones, Sign in Blood, are a great draw package for mono black
Toxic Deluge a great board wipe for when things need resetting, and you could follow with Thrilling Encore and get it all back
Syphon Mind is great draw if your playing with more players
Victimize - is really really strong, get rid of one weak thing for two of your best things
Yawgmoth's Will - lets you treat your graveyard like your hand for a turn
Lord Skitter, Sewer King makes rats
Marrow-Gnawer - lets you make lots of rats
Infectious Inquiry - good draw and gives poison
Ogre Slumlord - makes rats when your creatures or your opponents creatures die.
Tangled Colony - if your opponent has to block it or it dies to like Blasphemous Act will make you rats
Vraska's Fall removal and poison counter
Drown in Ichor removal and poison
Bontu's Monument all your creatures cost less
Rat Colony can have any number and they buff themselves, and cost less mana than Relentless Rats
Typhoid Rats deathtouch rat opponents wont want to block
Crypt Rats can board wipe but can also win the game if you give it infect, doesnt work with your commander because toxic specifies combat damage but does work if you give it infect and deal damage with it's ability.
Ratcatcher tutors rats, and gives you card advantage
Ashcoat of the Shadow Swarm - buffs your rats and lets you get rats back
Wave of Rats has blitz and reanimates itself
Ink-Eyes, Servant of Oni rat that steals opponents stuff
Nashi, Moon Sage's Scion rat that steals opponents stuff
Adaptive Automaton is a rat and buffs rats
Okiba-Gang Shinobi rat makes opponents discard and has ninjutsu
Filth gives all your rats swampwalk pairs well with Urborg, Tomb of Yawgmoth
Dark Ritual lets your get a head start or some extra mana a turn you need it
Tainted Strike instant speed infect, funny in multiplayer because you can use it on anyone's creatures.
Phyresis Outbreak gives poison and might kill opponents creatures
Feed the Swarm enchantment removal in black
Vat Emergence gets a creature back and proliferates
Spread the Sickness destroys a creatures and proliferates
Pact of the Serpent strong tribal card draw
Living Death swap graveyards with battlefields, great after sacrificing your whole board to draw cards
Skullclamp sack small rats for card draw
Thrumming Stone if you go the Relentless Rats or Rat Colony route
Coat of Arms your rats will be huge
Bolas's Citadel this card is dumb lol, lots of casting power and then you hit Gray Merchant of Asphodel and keep going lol
Icon of Ancestry kinda draw buffs rats...is ok
Contagion Engine proliferate twice each time you activate
Eldrazi Monument gives creatures flying indestructible and +1/+1
Grafted Exoskeleton gives a creature infect, great with Crypt Rats
Sword of Truth and Justice proliferate sword
Bad Moon buff all black creatures, opponents too
Phyresis gives infect again good with Crypt Rats

Vraska, Betrayal's Sting proliferate planes walker

Thanks for the suggestions, there are some great ones. I mean, that Nighthawk Scavenger is just a straight upgrade, and a serious one at that. I've tried Shriekmaw but I miss the sac-body Bone Shredder provides for Victimize. Buried Alive might be a good addition to get something into the graveyard, or are there any self mill cards I could use?
I'm gonna hold off on the more expensive cards for now, gonna wait a while to see if the group I'm playing with keeps playing Magic or if they're going back to boardgames.

I see a few Cathars' Crusade + Basri's Lieutenant + sac outlet and any other nontokeln creature with +1 counters Inf tokens and more
Phyrexian Altar + Haakon, Stromgald Scourge + 1 drop knight 2 life per etb ltb trigger some effects can reduce life loss/kill opponents
Note to brewer
I think Ashnod's Altar might be not worth it here. You might be better with another free sac outlet like Viscera Seer or more reanimation like Victimize
