All really solid recommendations SufferFromEDHD, I'll shake the base of the tree and see what falls off.
Ahead of the curve though, Emet-Selch, Unsundered Flip and Emet-Selch of the Third Seat are both here in the stead of Lier, but Lier does reinforce the playability of my entire deck. I like this. Lemme see if it benefits me to change it in such a way, because this might just influence how I build Vivi Ornitier as their own commander instead.

Sorry to spam your page, but just to clarify, all of the aforementioned pump spells only synergize with creatures that have infect. For creatures sporting the neutered Toxic mechanic, i'd highly recommend going the +1/+1 route primarily as it does help both creatures with Toxic and Infect - but splash in some of the really good explosive stuff like Hatred. You can literally hate out an opponent at random. Not to mention it has great synergy with Atraxa as she just gains you all that life back after spending it. No infect? No problem.
Alternatively, if you wanted to go some completely out of left field route, there is a creature in Edge of Eternities that does give all Artifact creatures you control Toxic 2 (technically). BUT this is a leak - so I can't link it or really discuss it at length. The leak does have some credibility is because on Mark Rosewater's Teaser for the set he does say under the "Things to Expect" section "poison card". So if you really desired it, you could go with an Artifact focused poison deck with an equipment like Grafted Exoskeleton.

It's about as fun as having a 'kick each other in the nuts' competition for game night, but I feel like this Commander would benefit from some stuff like Merciless Executioner, Plaguecrafter, and Accursed Marauder. The idea is you just play one of the many variants on the theme (there are many more), sacrificing itself each time, and then get it back to play again next turn. I had to limit myself in my Meren list to only 3 of them (and no Grave Pact enchantments), but depending on your play group it could be a significant inclusion.

I do have some ideas in mind when it comes to closing out a game with Poison counters. But you don't have to make it through a couple times - all it takes is once and it's the beginning of the end. Some traditional combo pieces would be:
And other spells of that ilk. Another synergy are +1/+1 counters. Forgotten Ancient, Hardened Scales, Branching Evolution, Ozolith, the Shattered Spire and The Ozolith are real powerhouses if you wish to go that route. This synergy does telegraph your moves more and you'd be less explosive.
But what to do when the table goes against you? I recommend stax. Tying up the resources of the table, forcing slower and less impactful turns on your opponents while the clock continues ticking. Cards like Astral Cornucopia and Everflowing Chalice help break parity as you will likely have access to more mana than your opponents while they're being held back. Some oldies but goldies would be:
- Winter Orb
- Static Orb
- Ensnaring Bridge (This has good synergy well with the aforementioned pump spells as you can still attack with low power infect creatures)
- Tangle Wire (A personal Fave as you can proliferate these counters)
- Parallax Wave (A personal Fave as you can proliferate these counters)
- Parallax Tide (A personal Fave as you can proliferate these counters)
Playing pretty much any of these cards will likely earn you some ire - but the idea is to only play these when you have poison counters on your opponents and you can reliably proliferate them at least once or twice a turn. The idea is that you've already got your opponents where you want them, all you'd need to do is simply wait while the Poison counters end them.

I recommend using Worthy Knight to combo with Elspeth in this deck. Werefox Bodyguard is also a must-have due to it being potentially another form of removal to have access to. I like seeing Get Lost in White decks recently, by the way.

41 lands is way too many. You probably don't need 2 of every basic land type but I would recommend some instants. Even just a Teferi's Protection or a Counterspell to keep your board state around for a turn cycle could do wonders for a deck like this.

You could try the old deck thinning Flagstones of Trokair trick. Drop 4x Plains and run the full playset, grabbing the Plains you need (works on Snow-Covered Plains too) and increasing the odds of drawing into something better next. It’s kind of pricey, and that’s all it accomplishes, but it’s one of those little adjustments that can increase performance.

This looks nutty and fun to play
that being said I think Words of Worship, Honor the Fallen, Y'shtola Rhul, Ruin Crab, Swiftfoot Boots could be cut in favor of some more raw lifegain. You already have a ton of protection and counters so I'm not worries about losing boots. I would consider cards like Drogskol Reaver, Rhox Faithmender, Suture Priest, The Wind Crystal, and a few more lands. Maze of Ith doesnt count so you are really only running 33 which is way too lean

Removed Cabal Coffers, Open the Graves, and Eliminate The competition because they just didnt generate any value in the deck. In their place I added a planeswalker to help generate buffable tokens and utility creatures to help card draw that is needed.

I would be really temped to build this deck to accommodate extra combat steps with Akki Battle Squad, or like Combat Celebrant, Scourge of the Throne, Port Razer, or Bloodthirster paired with an untapper, preferably Amulet of Vigor. Breath of Fury plus any evasive creature in this deck goes absolutely crazy.

In regards of manabase, I do have some shock lands/duals on my hand. The rule of proxies in my group is basically : if you own it, proxy it.
My group usually play bracket 3 and sometimes a low 4 and running not enough removal/counters. I do not mind spending, but i also don't want to make it a 5k deck.
Concerning the infinite combo with the poison/infect, my view on poison damage is pretty much like ''commander damage'', it's just another way to win but you gotta make it through a couple of times and not just get an infinite combo piece and it's done no matter what. Do you care showing me some combo you have in mind, i just might misunderstand what you're explaining. The reason i do not want to use such combo as Demonic Consultation + Thassa's Oracle is that we do not play that strong of decks in my pool and they don't enjoy it either. My pool doesn't run enough removal/counter to have these kind of strong game ender combo

You run 18 artifacts (plus creatures). It might be beneath your current passive life gain options but none of that doubles as a tutor. Chew on it. Agree with your Nomad analysis but still think Blighted could be a sneaky game ender.

This looks really fun. Both Boas seem weird, even though I get why they're here. My suggestion is to replace them with some copies of Scryb Ranger. It's a cheap flyer and can bounce you some lands before hitting in with Slayer.

Interesting take on mono red. A cursory glance has me worried at the lack of Turn 1 plays. You have Simian Spirit Guide for a free , true, but that’s not a guarantee. If you don’t have access to that on Turn 1, you can’t really do much and will fall behind. I’d recommend dropping a few cards to add some 1 drop spells. Plenty of decent goblins that cost , or even just Lightning Bolt or something.

Prosperity is kind of tricky to use. X can't be greater than the number of cards in your deck if you don't want to deck yourself, so if an opponent happens to have more cards in their library than you it won't be possible to deck them out. You will have still drawn your deck so you'll have a way to win regardless, but your opponents now have most of their decks in hand too so they'll potentially be able to use interaction to stop whatever you do next. Since you've drawn all of your cards that means you'll lose on your next draw step and they might just get to untap and win on their turns. The best way you have to offset all of that is to have Jace, Wielder of Mysteries on the board. That way you can make sure that X is high enough that Prosperity decks everyone and you won't lose.
It will be funny if someone bounces or destroys Jace in response to you casting Prosperity though. At that point, I'm sure that your opponents would prefer to force a draw than just let you take the clean win.
I run Drown in Dreams as one of my X cost draw spells, but I also run spells that let me get cards back from my graveyard like Eternal Witness and Noxious Revival so I can squeeze a little extra value out of the card. There have been a number of times where I've targeted myself with both the card draw and mill parts of the spell because I was really digging for a combo piece and I had a card in hand that let me get something back from the graveyard. I could also use Drown in Dreams to beat all of my opponents too. With Zaxara out and a mana combo active I can draw my deck and mill out an opponent, cast Eternal Witness to get Drown in Dreams back, and then cast the spell again to force one opponent to draw out their deck while milling out my last opponent.
