There are three main game plans in order, in this deck.

1) Put poison counters and spread them with "Proliferate". The ways to start this chain are Ichor Rats, Decimator Web, Skithiryx, the Blight Dragon, and a few others. Then proliferating the counters with Yawgmoth, Thran Physician, alongside other sources make for a challenging but satisfying victory.

2) Combo. The big one we are looking for is Yawgmoth, Thran Physician + Geralf's Messenger + any creature with undying. As long as we have at least more than HALF of our opponent's life total we can win. The other option is with a Blood Artist effect and any two undying creatures. Mikaeus, the Unhallowed makes easy work of this job.

3) Win with damage on the battlefield and reanimating big dudes. Buried Alive is the king of this.
