Hi, you've got a decent start here. If you wanted to get down to 60 cards, here's what I'd consider dropping:

4 Forest, Wild Growth and Verdant Haven. 20 lands should be plenty, especially with all of your Elf ramp cards.

Bountiful Harvest and Wurm's Tooth. Between Wellwisher and Essence Warden there's plenty of life gain.

2 Fog. The Druid's Deliverance fits your theme better.

Giant Growth, Titanic Growth and Trollhide. You could swap all of these out for an Overrun, Overwhelming Stampede or Ezuri, Renegade Leader if you have them.

2 Elvish Eulogist, Elvish Scout, Golgari Decoy and Skyshroud Archer. These creatures don't fully fit into what you're trying to do.

2 Moonglove Extract, Blessings of Nature, Cobbled Wings and Bower Passage. These just seem like filler.

It looks like your main goal is to stick a creature and then buff it as much as possible with +1/+1 counters. To improve the strength and consistency of the deck, I recommend cutting anything that isn't working towards that end (unless it's very efficient.) I'd identify those cards as Druid's Deliverance, Sundering Growth, Bountiful Harvest, Bump in the Night, Lair Delve, Blood Reckoning, Tablet of the Guilds, Primal Clay, Trestle Troll, and maybe Phylactery Lich (depending on how many artifacts you end up running in the end.) I'd also suggest swapping out Intimidation to stay modern legal, possibly with something like Bellowing Tanglewurm if you really really want the evasion. Your strongest cards that I'd want additional copies of are Abrupt Decay (budget permitting), Ring of Xathrid, Undying Evil, Slitherhead, and Death's Presence (assuming your meta is slow enough to run it.) It also looks like Tormented Soul is a critical piece that benefits the most from growing big, so having redundancy for it seems like a good idea. Changeling Outcast, Gingerbrute, and Silhana Ledgewalker could all be fine targets. For +1/+1 synergy, you have scavenge staples like Varolz, the Scar-Striped, Corpsejack Menace, and Lotleth Troll along with more general options like Polukranos, Unchained, Winding Constrictor, Hardened Scales, Hydra's Growth, Rishkar, Peema Renegade, Scavenging Ooze (which is particularly good in a sideboard), Drana, Liberator of Malakir, Hunger of the Howlpack, Forced Adaptation, and Llanowar Reborn. Finally, some cards I've had success with in graveyard-focused decks include Tragic Slip, Satyr Wayfinder, Grisly Salvage, Splinterfright, and Fauna Shaman. I think one thing to pay a little closer attention to is not 'what is the card' but 'what does it do for the deck'. Example: Blackblade Reforged is and equipement. But what does it do? It makes a creature bigger based on land. It cares about number of land in play. I would put it with other cards that care about land such as Bountiful Harvest, Multani, Yavimaya's Avatar, and Crash of Rhino Beetles. That gives you an idea of how many lands you want to have out.

Cards like Sakura-Tribe Elder and Yavimaya Elder I would put with your 'Land Tutors'. I tend to separate cards like Budoka Gardener  Flip into 'Land Drop'...cards that allow you to 'drop' or play more lands per turn. We're definitely going to be looking for more of those in the future! Plummet and Reclamation Sage are definitely 'Destruction' cards.

A few of the cards, like Wings of Hubris and Poison-Tip Archer, don't seem to have an answer...at least not a good one...for 'what does this do for the deck'. Archer likes lots of creatures to die. Does your deck want that? Not really. Wings wants to allow a big giant thing to hit someone really hard. Does your deck need help with that? Not really...most of your big things already trample.

On the bright side, this gives us more room for more goodies for the deck!

Nissa, Worldwaker would be a great addition, any number of basic land cards from your library, with omnath on the field, thats a big landfall move in a turn, and with the already crazy ramp you should be fine block for her, i would throw a Bountiful Harvest in to give some life gain

yea i could help you with that:Avenger of Zendikar-Gets tons of plants and powers them upBountiful Harvest-You get tons of lifeCollective Restraint-Depending on the colors your opponents play, they have to play 3-5 mana, so maybe a sideboard card.Draco-same deal as Collective Restraint, maybe a sideboard card.Exotic Curse-woo! a Drag Down enchantment!!!Dune-Brood Nephilim-You could make a deck based around this and kill with sand. Feed the Clan is pretty good, but it would be better if you could have a creature. Jace's Phantasm works well in a mill deck and has potential to trigger that 10 life from Clan. Jace's Phantasm could also give you another win condition. Brindle Boar could be a more expensive creature you could use, and he can be sacked for life.

Other possibilities that I highly recommend would be Healing Leaves and Blunt the Assault. Protective and gains you life.

Maybe Fruition or Bountiful Harvest, if you think those would work well.

Dosan's Oldest Chant is okay, and so is Hunters' Feast. More expensive, but they are okay. I would probably skip the Hunter's Feast and just choose Dosan's. Or skip both for the first two I recommended! Haha.

Sorry that I'm not great in this field. I don't really play green at all, and blue doesn't do too much healing. Hope I helped though!
