Goal of this deck is to get +1/+1 Counters on every creature and then find a Herald of Secret Streams to make all those creatures unblockable.

Incubation Druid provides Mana Ramp, especially with an added +1/+1 counter provided either by Pollenbright Druid, Anafenza, Kin-Tree Spirit, Llanowar Reborn or Dromoka's Command.

Mikaeus, the Lunarch and Stonecoil Serpent have Counters natively. Thus, they make Inspiring Call and Avatar of the Resolute more powerful. Vorel of the Hull Clade can turn Stonecoil Serpent and Avatar of the Resolute into huge threats with trample and reach to protect against anything that flies. Ajani Goldmane and Hardened Scales also help with that.

With the help of Inspiring Call and Tezzeret's Gambit it should be possible to find an Abzan Battle Priest in order to survive until you can get out Herald of Secret Streams to swing for the win. 

The sideboard has some protection against artifacts and enchantments and Heroic Intervention and Sheltering Word to protect your unblockable creatures loaded with counters.
