Monitor Okay, cool! So the tokens are for value just like mine. Okay here are some other suggestions then.
If you're not looking to fight with your tokens, I would suggest replacing Druids' Repository since it only really works well if you have multiple creatures attacking. Maybe throw in a Shaman of Forgotten Ways as additional creature ramp and a possible win con? It's easy to pull off if you have Itlimoc, Cradle of the Sun and a ton of creatures on your side of the board.
If you plan on throwing in Yawgmoth, Thran Physician , you can replace Underworld Connections with him I think. You can do it multiple times as long as there are creatures on the board and it has the additional function of making your opponent's creatures weaker or even removing Indestructible creatures.
If you are looking to board clear but not kill all your own stuff, I suggest putting in a Blasphemous Act paired with Vigor . Your creatures don't take damage and gain counters based on how much damage was prevented. You get 13 +1/+1 counters really easily for all your creatures, all the while clearing your opponents' boards. Vigor will die but it just gets shuffled back into your library. Or maybe an In Garruk's Wake , though it has a large cmc.
Another funny tip I have is that you can infinite with Mazirek, Kraul Death Priest and Ulasht, the Hate Seed with an Ashnod's Altar . It gives infinite colorless mana and infinite +1/+1 counters for all your creatures except Ulasht unless you have a counter/token doubler. And because you have Poison-Tip Archer , Purphoros, God of the Forge and Walking Ballista you have a lot of different options to win with this combo. Even Blasting Station can work if you have a token doubler out. The reason I like it is because it isn't a super cheap way to win. It isn't out right like "haha these two cards are out, i win". You need other tools to make it function. You can fetch them with Tooth and Nail if you were to put it in.
Other than that, I think your deck is pretty solid! 35-36 lands would be ideal in here for sure.

I strongly suggest the OG Ravnica legends, most of which are powerful but overshadowed by later printings within their guilds. Sure, two of them ( Grand Arbiter Augustin IV and Teysa, Orzhov Scion ) are among the more highly played commanders, but I was thinking more like Lyzolda, the Blood Witch , or Ulasht, the Hate Seed , or Savra, Queen of the Golgari , or Agrus Kos, Wojek Veteran . There's a lot of originality to be had in those decks, and a ton of customization and deckbuilding, plus you get the bonus of someone going "Woah, I've never seen a _ deck before!" basically every time you sit down to play.

- Mosswort Bridge wuerde gehen aber Kher Keep ist in der Strategie wahrscheinlich besser um excess mana gegen Token zu tauschen
- Fungal Reaches ist crap, bekommst genug Ramp durch Creatures und anderweitig
- Evolving Wilds & Terramorphic Expanse sind must pick in jedem multi-color deck, damit fixt man nicht nur Mana, sondern dnnt auch das Deck aus und erhht die Chance Karten die man braucht zu ziehen.

I like the idea of using Dragon Mask to recast Ulasht for more +1/+1's without the commander tax.

I see you running effects like Second Harvest , a Doubling Season seems like it'd help this list. I'd also expand upon the token theme and integrate it into your combo strategy... perhaps running Xenagos, the Reveler , one of the Garruk PWs, Primeval Bounty , and a few other generators. Ulasht, the Hate Seed , Mycoloth might be worth slotting in.
Also, Genesis Hydra can make for great plays throughout the early/mid/late game, and even if countered it's ability will still trigger on cast. Managorger Hydra can play nicely with doubling effects and gives you an early game threat to eat removal while you're building up.
You can definitely slot Food Chain and Birthing Pod in this deck, and would suggest you do. Eternal Witness should also find a way into the list, as well as ways to exploit our token mass, primarily Ashnod's Altar and Phyrexian Altar
For ramp, I'd throw in Joraga Treespeaker , Satyr Hedonist, Voyaging Satyr (you can combo this card with Nykthos, Shrine to Nyx for amazing ramp ability, drop the large majority of your taplands. I'd certainly recommend Gaea's Cradle , but it has a value more than double of your entire list, so it may be something you consider adding waaay later on.

@Joulskr, I've taken your suggestion and added some white. In doing so I've taken out Ulasht, the Hate Seed because despite it being a nice card, it was a little slower than I liked. I've added Cathars' Crusade because frankly, it's almost a game winner if it gets out due to high Saproling creation. I've also removed Beacon of Creation and Sprout Swarm replacing them with Fungal Sprouting, using cards like Wayfaring Temple and Cathars' Crusade I should be able to get some pretty strong creatures to make more Saprolings. I did check out the website and found some really awesome cards, however, many of the best weren't modern legal and I couldn't include them.

Alot of your nonbasic lands are not needed, and should either be removed or replaced with basic lands.
Not sure about Panharmonicon, Ulasht, the Hate Seed, Omnath, Locus of Mana/Omnath, Locus of Rage, and Dragon Broodmother. They don't add as to the "feed thromok" theme. I would want to add more ramp in the form of Birds of Paradise, Orcish Lumberjack, Arbor Elf, Llanowar Elves, and Chromatic Lantern, and Seething Song.
Removing the lands that enter the battlefield tapped and the more expensive spells for ramp spells should increase the speed and consistency of the deck.
