MAIN DECK:

IF YOU RUN IT IN LEGACY INSTEAD OF MODERN:

  • Putrid Imp (I'm in love with this card, even in decks other than Madness; unlimited manaless discard ability is seriously OP; with this in play you can just pay Madness costs on every spell - adding him would significantly change the value of quite a few cards should you concentrate on only paying Madness costs for no additional cost to discard.)

Oh! I liked the Phyrexian Arena idea! Instead of Rancor, Groundswell and Vines of Vastwood I would use Strength of Lunacy, Narcissism, Muscle Burst and Sylvan Might. Instead of Abrupt Decay or Maelstrom Pulse I would put Dark Withering. I'm doing a similar deck Threshold of Madness , maybe you can take a look to help me and take some ideias.

I would drop Strength of Lunacy. Getting it in with madness would be nice, but protection from white means all your white auras will come off.

SilentNSly Strength of Lunacy is meta choice ... and to be honest, relic of some past configurations.But I like it a lot. It can be complete blowout.

Honestly, I'm working on an Olivia deck as well, but you don't have many Madness options in this deck which is the main focus of Olivia's ability.

Let's see here, you can also had a sideboard of 10 cards, don't forget that!

Now, suggestion wise:

Strength of Lunacy

Psychotic Episode

Senseless Rage

Murderous Compulsion

Fiery Temper

Call to the Netherworld

Big Game Hunter

Avacyn's Judgment

Alms of the Vein

These are all viable for the sideboard or mainboard. Big Game Hunter is great if they like to pump creatures and I know it doesn't fit with the whole vampire theme, most of these don't, but you want to take advantage of +1/+1 your creature and getting a cheap spell too boot.

Burn from Within is also a good option for the sideboard if they have a creature you want to just remove completely from the game, or have spare mana to smack them in the face.

Flaying Tendrils > Biting Rain. I dont care if it has madness, tendrils is awesome. Exiling is extremely powerful.

Other madness creatures that can synergize with Lotleth Troll: Arrogant Wurm, Big Game Hunter, Gorgon Recluse, Grave Scrabbler, Muck Drubb, Reckless Wurm.

Other madness cards in general: Avacyn's Judgment, Gibbering Descent, Senseless Rage, Strength of Lunacy.

Fetches can help with your manabase.

With the large amount of, well, aggro in this deck, might I suggest Gaea's Anthem, Beastmaster Ascension, Whip of Erebos, or Bow of Nylea? Those could help.

Also, Having 2 discard enablers, one being creature specific, is not recommended. With discard being so heavily needed, more outlets are in demand.
