Flip your deck in the graveyard using Hermit Druid and a lack of basic lands and play Haunting Misery for lethal.

Some considerations:

  • You will have 4 Cabal Therapies after flipping with Hermit Druid so you can get rid of the druid by flashing a Therapy back for one more creature in the yard, possibly targeting yourself to discard the last creature(s)
  • We have barely more creatures than 20, with possibly an Elvish Spirit Guide exiled for mana, so any amount of lifegain will be an issue if we have not taken some swings. You could Living Wish for some extra creatures to discard, or board them in games 2/3.
  • Except for druid and the cyclers, all main-deck creatures can sac themselves to go to the yard for lethal count for Misery or to avoid an untimely Swords to Plowshares
  • Catch-all wishboard is available from Living Wish or Death Wish with artifact and enchantment removal, nonblack, nonartifact creature removal and discard of creatures or spells. Some additional utility lands (Glacial Chasm, Wasteland), ramp and fixing can be wished for. Also Bottle Gnomes for life gain (probably don't want to Death Wish for it though). All these slots are putative for now.
  • Cannot win earlier than T2 since Druid has summoning sickness. we also need 4 mana to combo. Note that you can exile both ESG for mana and still have 20 creatures in the yard.
  • What to Therapy for:
    • Creature removal: Swords to plowshares, Terror, Shock, etc.
    • Counterspells: Mana leak, Counterspell, etc.
    • Graveyard hate off the sideboard: Ebony Charm (instant speed that takes 3 or more cards off your Misery count), Planar Void
  • Example nut draw: Land, Petal, ESG, Land, Petal, Druid, Misery:
    • T1 Land, Petal, crack Petal: Druid
    • T2 Land, Petal, exile ESG, Crack Petal: Activate druid, Sac druid to Cabal therapy, play Haunting Misery for 21
  • Example T3 draw: Land, Land, Land, Druid, Blood Pet, Misery, *
    • T1 Land, Blood Pet
    • T2 Land, Druid
    • T3 Land, Activate druid, sac blood pet for the third mana of misery: 22 damage coming through (if you drew creatures, you can discard one off a flashbacked Therapy (or all if they had the same name ; alternatively if you drew a blood pet it's mana neutral and can be played out and sacced in the same turn)
