You didn't mention it in your description, so I'm not sure if you are already aware of this:You also have an alternate win-con with Exquisite Blood + Sanguine Bond or Exquisite Blood + Vizkopa Guildmage , they both go infinite and kill all opponents instantly. This works with Defiant Bloodlord too so you could find a spot for him if you want.

what about Bastion Protector?

You do have one infinite combo in your deck. Exquisite Blood + Vizkopa Guildmage will kill everyone else if you use the second ability of the Guildmage.

I feel like Avacyn, Angel of Hope takes your deck in a direction it doesn't need to go unless boardwipes are common in your playgroup. I see you run quite a few of them, and I'm wondering if Avacyn is the reason for it.

Speaking of boardwipes, False Prophet is a neat little boardwipe you can trigger if things get too hairy for you.

Reveillark is a fantastic card, but including your commander (and it's iffy to toss your commander into the graveyard) you only have 7 reanimation targets for it to hit. It's true that it hitting Karmic Guide is pretty awesome, but I don't know if it's worth the slot.

Unless you're running it so you don't have to crack your fetchlands Chromatic Lantern is a bad choice for the deck. With just the two colors being run it's usually going to be worse than Coalition Relic.

You should also definitely consider running Skullclamp the ability to grab an extra two cards off anything you sacrifice for 1 life and 1 mana is pretty great. Depending on how much life you usually gain Well of Lost Dreams could also find a good home in your deck.
