Who is the creator of one of the most insane artifacts in history? Tetsuo Umezawa or Toshiro Umezawa?

It's most likely a family heirloom, wielded by Toshiro Umezawa during the Kamigawa storyline.

Though maybe that both of them are related...

They are. Tetsuo Umezawa is descended from Toshiro Umezawa.

I get really bothered when I see Japanese names in their reversed order. Really, for the sake of accuracy the names should be Umezawa Tetsuo and Umezawa Toshiro.

Unfortunately that isn't correct in English terms CanadianShinobi, which is where the product is being marketed. If you wish to do it in Japanese context in an English language it should be this: Umezawa, Tetsuou.

If you read the stories it is Toshiro's "good" Jitte and he uses it to make glyphs out of blood (that's why it needs to deal damage to get blood on it) for his blood magic, if he rights it in blood it comes to pass very potent uni that Myojin of Night's Reach is (that is the uni that Toshiro fallows) on a side note I'm still pissed with how Marrow-Gnawer died
