When a permanent leaves the battlefield, it loses all counters it had while on the battlefield. The creature cards will reenter without any of their previous counters.

There are a few cards that remove permanents from the battlefield though, which remember things like counters on it, such as Tawnos's Coffin

Yes, it would get rid of the counters (Killing a Primordial Hydra
or Protean Hydra
for example).
Anyway, it works kinda funny with Allies like Ondu Cleric
and Hada Freeblade
, as those creatures check their abilities upon resolution, so if you have a bunch of allies, the would all enter the battlefield (say... 5) and the each of those abilities would be checked for n=5 times.

The counters would be removed, when the creatures come back it is treated as freshly arrived, anything that was on them beforehand is forgotten, equipment falls off, etc.
