Does the instants or sorceries exiled with Nivmagus Elemental have to be from your hand or can it be from the graveyard?

You can use double square brackets to link cards. It's the easiest way to make sure everyone reading your question knows what's going on: Nivmagus Elemental.

Neither. A "spell" specifically means a spell or copy of a spell that's currently on the stack. For example: if you've just cast a Doom Blade and it hasn't resolved yet, or made a copy of Lightning Bolt with an Isochron Scepter and it hasn't resolved yet, then you can exile that spell to feed Nivmagus Elemental. Cards in your hand, graveyard, or any other non-stack zone don't count as "spells".

Neither. It doesn't say instant or sorcery card, but spell. A spell is a card that is on the stack.

To use the ability, you have to cast an instant or sorcery card (turning it into a spell on the stack) and then exile it from the stack (put the card from the stack into exile, the spell doesn't resolve).
