Elixir of Immortality so if the combo gets countered you can try again. Also Briber's Purse and Dark Sphere for early game protection and maybe Jeweled Amulet for ramp.

I love this deck. You can also toss in Sickening Dreams.

I'm planning on making a deck very similar to this. Yours and @Son-of-Mace decks are good inspiration. I did however work out that a deck like this with Lake of the Dead and Dark Ritual can go off on turn 4, which is slightly ridiculous. Ideal start would be:

Turn 1: Play a Swamp, use that to cast Dark Ritual, use that mana to play Malaren.

Turn 2: Use Malaren to tutor for your Lake of the Dead. Play it or a swamp, depending on how close you want to play your cards to your chest.

Turn 3: Tutor for the Ad Nauseam. Drop the Lake if you haven't already, or a Swamp if you have. On the end step before your turn 4, tap the swamp for mana, then tap the Lake, sacrificing the Swamp for another 4 mana, then use that to cast Ad Nauseam at instant speed. Draw however much of your library that you need.

Turn 4: Tutor for the Skirge Familiar, drop another swamp, and use the same method as before to generate the 5 mana for it. Then you can Exsanguinate to your heart's content :)

And, if you're willing to wait an extra turn, you can use your turn 2 to instead tutor and/or play Boseiju, Who Shelters All to protect your combo pieces from being countered.

I think you could replace Skirge Familiar and Exsanguinate with Dark Sphere and Sickening Dreams. They are cheaper; money-wise and mana-wise.

Oh, sickening dreams will be aewsome! Actually better then the original one. Mox diamond and chrome, petal of lotus, dark ritual for beeing able to put your general out earlier. It will not kill you even if your draw all the library and can come in the oppening hand.Cast your general, tutor ad nausem and wait for a opportunity, cast it, draw cards until you get Boseiju and Sickening Dreams. If there is a blue opponent wait until boseiju untaps and win the game, otherwise play a swamp and discard 40 cards. Can use the elixir for back up, you cast Ad Nauseum with 5 lands, so you have 2 lands for Sickening Dreams and 3 to sac elixir and shuffle everything back. Pretty hard deck to beat. Ad Nauseum got countered? Search elixir in your next draw phase shuffle then back and tutor ad nauseum again for the win.

I understand that the deck would lose a bit of its elegance, but you could add Boseiju, Who Shelters All, Imp's Mischief, Dark Ritual, and Amulet of Vigor to make the deck a bit more resilient on its combo turn :)

You are then able to prevent a counterspell targeted at Skirge Familiar, and make your Exsanguinate uncounterable. Of course, they could have just countered your Ad Nauseam, but...

Might not be a bad idea to throw in a Duress and Distress to cover the possibility that a blue deck could cancel Exsanguinate and screw you over. I guess you could also throw in a Thoughtseize but I'm cheap and if your opponent has more than two cancel-type spells by turn three then I guess it's gg.
