Can you sac Tetravus with undying at the beginning of upkeep to create infinite tokens?

Combo I was thinking was Ashnod's Altar/Bloodflow Connoisseur/Grimgrin, Corpse-Born + Tetravus + Mikaeus, the Unhallowed: Unlimited Tokens. Maybe a huge Tetravus?

Your problem is that the Tetravus triggers trigger at the beginning of each of your upkeeps. That means you will have two triggers on the stack that you can stack any way you want, and that allow you to add or remove counters on the Tetravus that created the ability. So even if you do create, say, 3 tokens, then sacrifice it and return it to the battlefield with a total of 4 +1/+1 counters because of the Undying trigger, you won't get another two triggers because the beginning of your upkeep has passed. So while you do get to return Tetravus to the battlefield every time it dies if it didn't have any counters on it, you'll have to wait until your upkeep to turn the +1/+1 counters into tokens.

Another thing to consider is that Tetravus can only get +1/+1 counters via his ability from tokens created by that specific instance of Tetravus. So, if it dies and then returns, you won't be able to power it up with tokens created by the former Tetravus. This is because whenever a card references itself by name, it refers to that specific card, not any card with that name. Long story short, you will never be able to get more than a 6/6 Tetravus out of this - 1/1 itself, +1/+1 from Mikaeus, +1/+1 from the Undying counter, and +3/+3 from it's own ETB.
