Antidotes do exist in M:tG, although usually flavored as some sort of life gain. Pharika, God of Affliction and the Witherbloom school of Strixhaven both have dedicated medicinal practices. And cards like Voracious Cobra and Serrated Scorpion indicate that some species have very potent poisons. And that's not too unusual, plenty of real-world biological and mineral poisons don't have an antidote.

Another point is that aside from the oddballs like Fynn, the Fangbearer and Persuasive Interrogators, the biggest source of poison counters in-story is Phyrexian oil, which until relatively recently was explicitly permanent and incurable. And it would be confusing and awkward to represent different poiosn with both poison counters, representing incurable poisons, and a hypothetical "venom counter" or something representing curable poisons. Gameplay takes precedence over flavor, so the simplest solution is that poison that affects players is incurable.

Also, deathtouch and wither can also represent incurable poisons, so it's not just poison counters.
