Does Tormenting Voice's additional cost effect anything when cast with Isochron Scepter

If I cast a Tormenting Voice from under an Isochron Scepter, do I still need to discard a card?

Yes, you do. Isochron Scepter says you cast the copy "without paying its mana cost", so that's the only part of the cost you're getting a free pass on. Any mandatory additional costs must be paid, and any optional additional costs can be paid if you want to pay them (which is why Isochron Scepter+Orim's Chant is a thing).

You can't imprint Tormenting Voice onto Isochron Scepter because you can only imprint instants onto Isochron Scepter and Tormenting Voice is a sorcery.

If Tormenting Voice was an instant:

From Isochron Scepter's gather rulings:

12/1/2004: You don't pay the spell's mana cost. If the spell has X in its mana cost, X is 0. You do pay any additional costs for that spell. You can't use any alternative costs.

Discarding a card is an additional cost, so you would have to pay it.
