Does Hapatra, Vizier of Poisons make a snake token per -1/-1 or just one per turn?

Having trouble with the mechanics of this card. Need a little help. Thanks.

Neither, it creates one token for each instance of putting -1/-1 counters on a creature regardless of amount of -1/-1 counters put in instance.

A single instance of putting -1/-1 counters on multiple creatures will create a token per creature though.

Example:

Your opponent controls 4 5/5 creatures, you control just Hapetra.

You play Black Sun's Zenith for X = 5 putting 5 -1/-1 counters on every creature.

Everything dies and you get 5 1/1 deathtouch snakes. Not just one, and not one for every counter that was placed (5 x 5 = 25).
