What is the designated area of the card "Totally Lost"?

The card Totally Lost doesn't specify where you can get target permanent from. Is the card only limited to graveyards or does it also apply to libraries and exile piles?

You have to choose the cards on the battlefield. It would specify if it could be from graveyards.

If an effect uses the name of a permanent type (or the word "permanent") and doesn't also say "spell" or "card" and mention another zone, it means it's only talking about something on the battlefield. Totally Lost can only target permanents on the battlefield.
