Primal Vigor and Hydras and Copies

Primal Vigor says whenever a counter is PLACED on a creature, double it. What happens if the creature enters WITH counters. ex: Would Kalonian Hydra enter with 4 +1/+1 counters, or 8. Another question about Primal Vigor: lets say you have 1 Master Biomancer, and 1 Primal Vigor. I cast Stolen Identity on Master Biomancer. Would it come in as a 6/8, with 2 counters from Master Biomancer, and 2 counters from Primal Vigor. And also since they are tokens, does one enter as the 6/8, and the other one as a 18/20 because of 6+2 is 8 counters on the other Master Biomancers, doubled from Primal Vigor?? Or are they both just 6/8's?? I know this question is pretty complicated, but when I had all those cards on the field, i had no idea how to distribute the counters.
Simultaneosly another Master Biomancer would come in, receiving 16 +1/+1 counters from both Master's, if I'm correct about priorty and stack. But either way, another Mater would come in due to Primal Vigor

"Primal Vigor affects permanents that enter the battlefield with a certain number of counters. For example, if a creature would normally enter the battlefield with three +1/+1 counters on it, it will enter with six +1/+1 counters on it."
The Master Biomancer tokens would enter the Battlefield simultaneously and receive 4 +1/+1 counters from Master Biomancer, doubled by Primal Vigor.
