Do you lose counters if a creature dies?

Lets say that I load up a Tormented Soul with +1/+1 counters, then it is killed somehow. Now I want to use Ghoulcaller's Chant on it. Will it keep the counters?What if before it was killed it had Unhallowed Pact on it, would it keep the counters then?

No, because whenever a permanent changes zones it will lose all counters on it unless otherwise specified like Skullbriar, the Walking Grave .

Tuned is right. More generally, when a permanent leaves the battlefield then returns, it comes back as a completely different permanent. I like to think of it as being a separate card just with the same name.

The only way to "rescue" the counters on your Tormented Soul would be to regenerate it or prevent whatever killed it in the first place, as then it wouldn't change zones and thus lose its counters.
