Moonmist and Liliana, Heretical Healer Flip

Someone brought up that Moonmist would transform Liliana, Heretical Healer into a Planeswalker. But wouldn't that kill her? Because I was pretty sure that the Planeswalker has to enter to have it's loyalty counters placed on it.

You're correct. Planeswalkers enter the battlefield with loyalty counters on them, but a permanent that becomes a planeswalker without having any preexisting loyalty counters on it will be put into its owner's graveyard as a state-based action. Becoming a planeswalker doesn't automatically place loyalty counters on something. This is why the new Liliana dies, goes to exile, and then reenters the battlefield instead of simply transforming.

306.5b A planeswalker is treated as if its text box included, "This permanent enters the battlefield with a number of loyalty counters on it equal to its printed loyalty number." This ability creates a replacement effect (see rule 614.1c).
