If I cast Careful Consideration with Notion Thief on the field?

If I chose my opponent to Draw four cards at the end of their turn, I would draw those four cards because of the Notion Thief, but who would discard 3 cards? My opponent because (s)he was my target? Or me because I drew the cards?

The opponent would still discard if able.

Careful Consideration prescribes a list of actions for the target player. Notion Thief defines a replacement effect that applies to one of those actions. The replacement effect doesn't change the target of Careful Consideration, and it doesn't change the prescription Careful Consideration creates. It only changes what is says it changes.

You'll draw, but the opponent will discard.

Thank you, I knew the opponent would lose life with Sign in Blood but wasn't sure with Careful Consideration. I think the 'then' wording rather than 'and' is what confused me.

"Then" means the instructions happen in a certain sequence. "And" means they happen at the same time.
