Let's say I activate Steel Overseer with three servo tokens in play; will Animation Module trigger 4 times or once?

It will trigger once for each creature receiving a counter. If instead two counters were being placed on each of those four creatures (maybe you have Hardened Scales), then it would still trigger only once for each creature.
