Graft creatures/Hydras and double counters

So I was wondering if effects like Corpsejack Menace and Doubling Season would effect creatures with Graft or Hydras that come into play with X +1/+1 counters on them or do the counters materialize before they hit the field?
Also wondering is you move a counter from say Vigean Hydropon to another creature via graft and one of the above cards in on the field does that counter become 2 since I'm placing it on a new creature or does it stay as 1

Counters being placed on a creature as they come in are doubled by the mentioned effects.
Counters being moved from one creature to another count as counters being placed on the creature they are being moved to and are also doubled by the mentioned effects.

Yes they do. Hardened Scales as well.
