How Does General Tax Work in Commander?

Does tax accumulate for every time a commander is sent to the command zone or is it for every time the commander is removed from the game, excluding bounce.
E.g. Tymaret, the Murder King dies seven times in a game, each time returned from the grave by sacrificing a creature before an opponent plays Bojuka Bog exiling the grave with timmy inside it...
Does this accumulate a commander tax of 14 because he died 7 times or is it a commander tax of 2 mana because this is the first time he's been sent to the command zone?
So in your situation, Tymaret, the Murder King dies 7 times and you choose not to put him into the command zone, and then Bojuka Bog comes along and exiles it and you choose to move Timmy to the command zone. In that case it will only cast 2BR. From the EDH Rules: "As an additional cost to cast a Commander from the command zone, its owner must pay 2 for each time it was previously cast from the command zone."

I thought if you summoned him from your hand, no matter what, his casting cost remains the same unless casting it from command zone..

Commander Tax is this:
"A Commander costs (2) more to cast for each time it has previously been cast from the command zone"
It doesn't care what happens to it at any time, except when you cast, how many times you've cast it before.
