What is a permanent spell?

I really struggle with Lithoform Engine . It made me realise that I have some grey zones and I hope that the following questions can be answered:
1) What is a permanent spell? 2) Is any card in the stack a spell? 3) Am I right in the assumption that once a card is in the battlefield, it is not a spell anymore? In other words, only cards in my hand and in the graveyard are spells? 4) Is a creature or any other card type that was exiled a spell?

Spells exist only on the stack, when they are cast and waiting to resolve. So when you cast a card or a copy of a card it becomes a spell on the stack. Most activated and triggered abilities also go on the stack, but they are not spells.
A permanent spell means everything that would itself become a permanent after it resolves. Like an Enchantement, Artifact, Creature, Planeswalker (NOT a sorcery that produces tokens!). Lands are permanents, but they are not spells and they are not cast, so they won't go on the stack.
