Yes, as soon as something targets the illusion, a triggered ability goes on the stack that forces you to sacrifice it. So even if you respond to the thing that targets it, the trigger is already on the stack.

There are a few ways to counter triggered abilities that I think might be able to save it, but I can't remember what cards those are.

It's more useful to understand why this is true rather than that it is true.

When a phantasmal creature like Phantasmal Bear MTG Card: Phantasmal Bear becomes the target of a spell or ability, its ability triggers and it put onto the stack. Since abilities on the stack are treated independently of their sources and the circumstances that caused them to be put onto the stack, the triggered sacrifice ability will still exist even if the spell or ability is countered. In fact, the sacrifice ability will resolve before the spell or ability that triggered it anyway.

StifleMTG Card: Stifle and VoidslimeMTG Card: Voidslime are both ways to counter abilities, and therefore can be used to save an illusion from being sacrificed.

It should also be noted that because the illusion will be sacked before the spell resolves, if the illusion is the only target of that spell, the spell or ability will be countered.

For example a Chandra's OutrageMTG Card: Chandra's Outrage on a Phantasmal Bear MTG Card: Phantasmal Bear will not result in two damage to the dragons controller.
