If I block Etali Primal Sickness (12/12) with a 4/6, how many poison counters do I get?

The card reads: Whenever Etali deals combat damage to a player, they get that many poison counters. (A player with ten or more poison counters loses the game.)

Its seems if I only get 6 damage, then I should get only 6 poison counters.

Your reasoning is correct, but I think your numbers are off. In general, an attacking creature with trample deals enough damage to any blockers to kill them, then deals the remainder of its damage to the defending player.

Etali P-Sick has 11 power, so it deals 6 damage to your blocker and the remaining 5 damage to you. This triggers its poison ability, which puts poison counters on you equal to the combat damage it dealt to you. It dealt 5 damage, so you get 5 poison counters.
