How would Mistcutter Hydra and Primal Surge work together?

I don't know if Mistcutter Hydra would enter the battlefield as a 0/0 and then die, or if it would somehow enter with some +1/+1 counters on it.

Your first instinct was right. Barring any other cards being involved, Mistcutter Hydra only enters with +1/+1 counters on it equal to X, and the only way X gets a value other than being zero is by being cast. Mistcutter Hydra would enter the battlefield as a 0/0 and be sent to the graveyard as a state-based action.
