I know it forces you to mill till you hit a land. Does the land go to the graveyard too?

Yes. Trepanation Blade says that "the revealed cards" are put into the graveyard, and that land card was revealed. The land also counts toward the "+1/+0 for each card revealed this way" total.

The trigger is worded as "Whenever equipped creature attacks" which typically only happens once per combat, which typically is once per turn- the trigger actually happens before blockers even and well before damage. It doesn't matter if anyone double strike, the result is the same in this case. Double strike typically modifies abilities that involve combat damage (eg. "whenever equipped creature deals combat damage to a player...")
If you want the trigger more than once a turn, generally you either need more combat phases so you can declare it as an attacker more than once (such as Seize the Day) or something to copy triggered abilities (such as Strionic Resonator).

Trepanation Blade triggers and resolves during the declare attackers step of the combat phase, it will resolve before the declare blockers step. An attacking creatures with skulk equipped with Trepanation Blade will have its power increased by X and can be blocked by any creature with power lower or equal to its modified power.

Yes. From the card's text: "defending player reveals cards from the top of their library until they reveal a land card" and then "That player puts the revealed cards into their graveyard." Since the land card was among the revealed cards, it goes into the graveyard as well.
Incidentally "The creature gets +1/+0 until end of turn for each card revealed this way." means the land card also counts for the power bonus.
