Does Sabertooth Cobra ability stack?

when I attack an opponent more than once, does the upkeep cost stack on them?

Yes it does. If you give it double strike somehow, its ability will trigger twice (once for each instance of combat damage). (Note that it triggers off any damage, not just combat).

When the triggers resolve, they each create a delayed triggered ability (DTA) that trigger on the affected player's next upkeep.

each DTA will trigger and the opponent will get another poison counter for each DTA unless he paid (for each individual DTA) prior to the DTA's actually triggering.
