Do I need anything on the field to use cards like Smallpox?

Simple question, but it's always good to be sure!
With cards like Smallpox which don't actually designate or require a target, can they be played while I control no lands, no hand and no creatures (assuming I got 2 mana from elsewhere)?

Yes, you can cast it. You don't need to fill all of the effects in order to cast the spell. The only normal requirements are timing and mana, since as you mentioned you don't need to specify legal targets for a spell like Smallpox. It's kind of the whole point to use a spell like Smallpox against your opponent when it's the least symmetrical on your side.
