When I cast Citadel Siege, and chose Khans. Does it mean I put +1/+1 on target creature EVERY beginning of my combat or is it good for one time?

It happens each time you begin combat on your turn. The spell is a permanent, so it remains in play once cast successfully. The ability in question is a triggered ability of Citadel Siege, so it will continue to occur whenever its trigger condition is met (beginning of combat on your turn arrives, choose Khans for its ETB trigger way back when).
It's worth noting it isn't an optional trigger, and you can't skip your combat step (even if you don't attack).
