what does transform actually do

when it says "transform this creature" what the heck does that do?

It means you flip the card over to reveal the other face, this is for the double-faced cards in Innistrad, particularly the werewolves.

You take the card and flip it so the normal face is down. On the back of the card, where the traditional back design would normally be, there is another face to the card. That represents the card form of what the original face has "transformed" to.

Ive wondered, doesn't this kinda announce a couple of the cards you have to your opponent? i mean yea you can fold the cards together but still.

That's why you sleeve them. Wizards has amended the rules to state that an unsleeved transform card being visible in the hand, library, etc. does not constitute a violation for revealing excess information or looking at hidden information.

If you do not play without sleeves, your transform cards will likely be visible to your opponent at some point. If you play with sleeves, you can use the checklist or remove the card from the sleeve to indicate transformations and show what form is currently on the battlefield.

@Epochalyptik, the new rule making double-faced cards open information only applies to drafting. When a double-faced card is in a deck, it has to be either sleeved or represented with a properly filled out checklist card.
