When teysa and black creatures are sent to the graveyard at the same time, does her second ability trigger or not?

You sacrifice all three cards at the same time, so Teysa, Orzhov Scion 's ability will trigger for as many of the other two as are relevant. Note that the ability won't trigger of Teysa, Orzhov Scion itself is sacrificed because the ability specifies "another."

Yes it will. Multiple objects change zones (usually the battlefield) they all "see" each other going there. So if you get board wiped and 2 other black creatures, Teysa, Orzhov Scion will trigger twice.
