Land - Creature?

If play say a Hydroform and use that land-creature to block an opponents creature and my land-creature is destroyed does my land card go into the graveyard? Or do I return it to the rest of my mana

Yes; as it is now a creature, after lethal damage has been marked on it, it is sent to the graveyard as a state-based action.

The land card will go in the graveyard. Because since you turned your Land into a Creature, and your Land/Creature is blocking there attacking Creature, then when your Land/Creature dies, it is still a Creature and will be placed in the graveyard.
