Smallpox and Basking Rootwalla?

If I cast a Smallpox with no creatures on board and discard Basking Rootwalla and cast it off of Madness for free do I have to sac it to Smallpox?

No. Madness allows you to exile the creature instead of putting it into your graveyard during a discard. You then have the option to cast it for its madness cost.

You don't just put the creature directly onto the battlefield the instant you would discard it. For that kind of effect, you'd need something worded like Loxodon Smiter .
