If a equipment becomes a creature

Lets say my Sword of the Meek is turned into a creature from Ensoul Artifact, if the sword is currently equipped will it fall off of the creature?

Yes. Creatures can't be attached to anything. If an equipment or an aura becomes a creature, it falls off of whatever object it was attached to.
