how does the exhalted trigger work?

do you get a +1/+1 counter for every card that says exhalted on it or do you get only +1/+1 counter for every card like if you had rafiq and finest hour out would that mean you would get two +1/+1 triggers

You do not get a +1/+1 counter in the slightest - you get a temporary +1/+1 bonus that goes away at the end of the turn. This is an extremely important distinction given the fact counters last an indefinite time and there are myriad cards that interact with counters specifically.

However, that aside, each instance of exalted is independent of one another and will trigger individually. So, if you have three creatures with exalted, and then have one creature attack alone, that attacking creature will get a total of +3/+3 until end of turn due to the three independent triggers of exalted.

Whenever a creature you control is attacking alone (declared as an attacker), it will receive +1/+1 until end of turn for each instance of exalted on permanents you control. Cards can have multiple instances of exalted and each instance will trigger separately.

This can be seen on the reminder text of cards like Sublime Archangel .
