Does Kazarov, Sengir Pureblood activate multiple times during the damage phase of combat?

Would Kazarov, Sengir Pureblood get 3 +1/+1 counters if an opponent blocked with 3 creatures during combat and they all took damage, or would it be all at once and only trigger Kazarov once?

It will get 3 counters.

Kazarov's triggered ability looks for the event "a creature an opponent controls is dealt damage". In your example that happens 3 times during the combat damage step. It's fine if the events happen simultaneously. The triggered ability still sees them as separate events and will trigger for each one.

Note that this is also addressed in one of the rulings notes on Kazarov's Gatherer entry:
4/27/2018 Kazarov's triggered ability triggers once for each creature dealt damage at one time.

Kazarov, Sengir Pureblood will trigger for each creature that took damage. If it only triggered once for multiple creatures taking damage it would be phrased "Whenever one or more..."
