As far as decks used, one of them was Azusa, Lost. The other was a fairly casual Rhys the Redeemed list that probably doesn't even break the 50% mark as far as optimization level.

It isn't difficult to handle Every Last Vestige Shall Rot if you're just defining behavior. You need a rule that states "Nicol Bolas always pays full costs for effects" and errata Every Last Vestige Shall Rot to have a limit of INT_MAX for X (or declare a finite, but very large amount of mana for Bolas to be able to spend each turn). This rule also prevents cards like Fade Away and Flow of Maggots from causing problems for the Bolas AI. If you add this rule, you'll probably want to ban Killing Wave, but that card should generally be banned from any format where at least one player is autonomous.

As an aside, it's possible under the current ruleset for the first player to get targeted by Behold the Power of Destruction and then hit by Nicol Bolas, which could knock a player mostly out of the game. Is this something worth changing the rules to avoid?

Ok, I played a couple games of this with two non-Bolas players, and it seemed WAY too easy. Many of the schemes were underwhelming, and none of the optional rules regarding Bolas's life total were relevant because he didn't disrupt the players enough to stop them from smashing for 100+ damage (no loops involved, either). Cards like Look Skyward and Despair, Approach My Molten Realm and The Fate of the Flammable were either dead or actively bad for Bolas.

Also, Every Last Vestige Shall Rot doesn't seem banworthy. It's a Behold the Power of Destruction that tucks instead of destroying, but that's not necessarily going to end the game.
