What exactly does Hand to Hand do and how would I use it?

Never thought I would be this clueless about a card with such simple text:
Hand to Hand
So I understand this stops Maze of Ith from working, but for the second half, what kind of instant requires an activation cost? And when when would this card work in your favor? I'm looking for examples because I'm having a total brain fart when looking at this.

Cards are handled based on their Oracle text, not their written text. Gatherer has the current wording:

During combat, players can't cast instant spells or activate abilities that aren't mana abilities.

This wording makes it pretty evident what Hand to Hand stops. You can't cast any instant spells during combat.
