Plus one and minus one counters.

If I have a plus one counter and my opponent puts a minus one counter on my creature do I keep both or lose them? What really happens?

They negate each other, creature is left without counters

704.5q If a permanent has both a +1/+1 counter and a -1/-1 counter on it, N +1/+1 and N -1/-1 counters are removed from it, where N is the smaller of the number of +1/+1 and -1/-1 counters on it.

It you're in a wonky situation with something like a +2/+2 counter or -2/-1 counter they stay, but the standard +1/+1 and -1/-1 remove eachother as per the rule Gidgetimer cited.

unequal counters like -2/-1 aren't used anymore. in fact any counters other than + and - 1/1s haven't been used in ages and likely wont again.

Old cards that use uneven counters are still legal in 3 out of 6 major constructed formats so the fact that new cards that use them are no longer printed is irrelevant. No format was cited so all information relevant to any type of plus or minus counter is relevant to this question.
