  1. Squire is enchanted with Cloak of Invisibility and it phases out. I then use Simic Guildmage second ability to move cloak of invisibility to another creature. Does squire phase in next upkeep or stay phased out?

  2. So If I enchant Eager Cadet with Vanishing and phase it out and then move vanishing to another creature, does eager cadet phase in at the next upkeep?

  3. do equipped equipments fall off phased out creatures?

702.25f When a permanent phases out, any Auras, Equipment, or Fortifications attached to that permanent phase out at the same time. This alternate way of phasing out is known as phasing out indirectly. An Aura, Equipment, or Fortification that phased out indirectly wont phase in by itself, but instead phases in along with the permanent its attached to.

The auras will not be on the battlefield to move after the creature they are attached to phases out. Equipment also won't fall off since it phases out with the permanent it was attached to. However to answer the spirit of the first two questions; objects do not need to have phasing in order to phase back in on the untap step. All objects that were phased out under the control of the active player as the turn started will phase in.

702.25a Phasing is a static ability that modifies the rules of the untap step. During each players untap step, before the active player untaps his or her permanents, all phased-in permanents with phasing that player controls phase out. Simultaneously, all phased-out permanents that had phased out under that players control phase in.

Yeah, basically you can't move the auras because they phase out with the creature they're on. Additionally, anything phased out phases back in on the untap step. It doesn't matter why they phased out, or if they have phasing.
