Mikaeus and an Eldrazi Titan

If I have both Mikaeus, the Unhallowed (or something like Cauldron of Souls) and an Eldrazi Titan like Kozilek, Butcher of Truth on the battle field what happens if:
A. Someone Go for the Throats Kozilek.
B. Someone boardwipes.
With both scenarios, I know my graveyard is shuffled into my deck, but in A. does Kozilek, Butcher of Truth un-die, or does he get shuffled in too?
And in B. does Mikaeus, the Unhallowed get shuffled in as well?

The answer is it is up to you what happens.
You will have two triggers that you control going on the stack at the same time, so you get to choose how they are stacked. You can stack them so that the Kozilek triggers resolves first, in which case he would be shuffled into the library along with your graveyard and then the undying trigger would resolve doing nothing, or you can choose to have the undying trigger resolve first, in which case Kozilek will return to the battlefield with a +1/+1 counter and then you will shuffle your graveyard into your library.
In case B Mikaeus will be shuffled when the Kozilek trigger resolves.
