Is there a card that removes poison counters, or any type of counters from players... ?

Yes, but it doesn't do what you want it to do. There are two ways to remove poison counters. The first is ending the game, which if you want to keep playing means using Karn Liberated to restart the game. The second is with Leeches, which as it deals damage equal to the number of poison counters removed, it would put them right back on if you had Phyrexian Unlife and 0 or less life.

You can only sacrifice permanents. Once a sorcery like Leeches begins resolving nothing can interrupt that process. Either the entire spell get's countered or none of it does. What you could do is use something like Dromoka's Command to prevent the damage. The real issue is you would need a way to continually recur leaches to essentially infuse yourself with 9 or less life. There are easier infinite life combos out there, and this one loses to taking 10 damage in a single blow.

Got it: cast Leeches, then activate Circle of Protection: White targeting Leeches, preventing the damage, and removing the counters.

That would do it. Note however that you're now dealing with a 4 card combo that's only legal in Vintage and Legacy, gets broken up by artifact removal, and straight up loses to Enchantment removal or taking 10 damage in a game round. Still, if your just playing kitchen table with friends, it's not the worst combo I've seen.
