How does Neko-Te damage to players work?

Neko-Te says "Whenever equipped creature deals damage to a player, that player loses 1 life."

Let's say that I'm attacking my opponent with Lightning Elemental which is a 4/1 creature. The Lightning Elemental is equipped with Neko-Te.

Does this mean that the Lightning Elemental will deal 1 additional damage to the player, making the total damage it deals 5, or does this mean the Lightning Elemental will only deal 1 damage to the player?

Neither.

Neko-Te has an ability that triggers when the equipped creature deals damage to a player. In your example, Lightning Elemental will deal 4 damage to your opponent, which will trigger Neko-Te's second ability. When that triggered ability resolves, your opponent will lose 1 life. The Elemental is not dealing an extra damage, nor is its damage replaced.

It's important to note that the life loss from Neko-Te's second ability is not damage. If something cares about a player being damaged, this ability will not trigger that. For instance, Sower of Discord will not trigger from the Neko-Te life loss, only from the damage the Elemental did.
