Does Thalia's Lieutenant trigger on Human flips?

I've been working on building a white weenie deck, my first, and was wondering if Thalia's Lieutenant +1/+1 would trigger when Hanweir Militia Captain Flip flips into Westvale Cult Leader Flip?

If you play two Thalia's Lieutenant together do they enter with two counters each?

It depends. If you cast both of them like you would regularly then the first would get the two counters. But if you play something like Collected Company or something similar that brings out both Thalia's Lieutenant then I would say they would because they would see each other coming out at the same time.

No.
An object only changes zones if it moves into a new zone from a different zone. "Enters the battlefield" is a zone change that requires moving from a non-battlefield zone to the battlefield.
When an object transforms, it's still the same permanent. It just changes characteristics. It doesn't change zones.
