Combustible Gearhulk ETB triggers. I respond by destroying the gearhulk. ETB resolves and I choose to have my opponent mill the top cards. Can gearhulk deal damage to me even if he is in the grave? Sa me if exiled?

Combustible Gearhulk ETB gives you the option of allowing a draw or milling with damage from the gearhulk itself. If the gearhulk is in the grave or exiled can it still deal damage?

Yes it can still deal damage. Once an activated or triggered ability is put onto the stack, getting rid of the source won't go back and un-activate/trigger it. If the game needs any information about the source as the ability resolves then it will use the last known information from the moment the source was last on the battlefield.
