What does the title next to your username mean? Why do some people seem to have custom ones? How can I change mine?

You can change your title to a custom one by upgrading your account! Same with name styles and many other site features.

I believe you also need to be ranked 100 or higher,

Maybe it's rank 99, or it takes time for the option to appear?

I think Simon_Williamson is right, you also need to be upgraded too.

It's hard to know exactly what the rules are, it's one of T/O's hidden mysteries. :)

Yeah you can change the rank with a paid account. I am not sure how it interacts with the rank, back when I was in low 30s it was gone but now the rank is back.

Yes, paid accounts in the top 100 can change their title.

Wait, what? I knew that there were different levels about this, but I am a 'Deckspert' with an unpaid account. Top 10, I believe Simon_Williamson is as well, but he's still a Deckling.

Deckling changes to Deckspert after certain conditions are met. I believe it has to do with deck score/average deck rating.
